I use Bogen amps - page 1 for inside, page 2 for outside. I believe ICM 330 is for page 1, ICM 331 is for outside; I know ICM 33* is for both. Try ICM 33* - that works the best here, 330 is low for some reason. I used shielded cable with the center conductor to tip, shield to ring at the amp.
My system is a -4, and also shows 196 as not valid, although I don't know where you got that.
