I'm glad you didn't hurt yourself and you probably won't be the last to
experience a similar accident. All three mistakes you listed violated a
one line piece of advise I've posted more than once on the net. "Never
take an airplane to the runway unless both the pilot and the airplane
are ready to fly". Get it repaired and get back out there when you're
BOTH ready.
