Lots of examples of bad charts out there.
In the case of the US minesweeper incident,  though,  that reef was charted
on some charts as the Greenpeace boat had wrecked there a few years earlier
and had reported it.   But the US Navy wasn't using those charts as they
had their own.  (thus reinforcing the 'open source versus proprietary'
debate.
