I'm having a bit of a time converting a DocBook file to a pdf while
maintaining the profile look. I have a .profiles file with some entries

I selected the .profiles file in Conditional Processing profiles. Then
for conversion, I'm going to DocBook -> Convert Document -> Convert to
PDF. I have tried adding a profile.revision parameter with Revision as
the value (a selection in my document) with no luck.

Just guessing: did you select the profiling XSL stylesheets prior to using "DocBook -> Convert Document -> Convert to PDF"?

This setting is done once for all and not each time you use "DocBook -> Convert Document -> Convert to PDF". See attached screenshot.
