Hi, Hussein.

Our users open and the index.html so that they can navigate
using links. The Webhelp files are put out on a shared server, but
I believe they are "pushed" out (distributed) to a finite set of
users (not viewing it from on a website).

Does that help any?

Thanks,
Marilyn

I'm sorry but I don't understand "complete paragraph numbering scheme".
Do you mean part/chapter/section numbering?

I'm sorry but I don't understand "as viewed by the index file". Do you mean
the "Contents" tab?

I'm really sorry but I don't understand.

An annotated screenshot of the WebHelp generated by XXE would help.
Sending us the PDF file (or a screenshot of a part of the PDF file)
would help us understanding what's wrong with WebHelp by comparing the
two outputs.
