        Or you could just set your reminder for the time you want to first be 
alerted and include the actual appointment time within the text of the 
reminder.

                The only thing is that if you want to be reminded a certain 
time prior to your appointment time, you have to open the reminders app after

appointment time. The options are:

                5 minutes before

                15 minutes before

                2 hours before

                1 day before

                Custom – this allows you to select, for example, 3 hours or 5 
hours before, but you can only do full hours, something like 4 hours and 30 
minutes before is not supported.

                I have so far not found a way to set a reminder with Siri for a 
particular time and include the information about a 2 hour early reminder.
