    I don't know if this will help: if you are shooting a piece of paper in 
portrait, with the phone also in portrait, then you lift the phone equal to the 
length of the longest side.

   So, if the paper is, say, 12 x 8, you will lift it 12 inches. You don't have 
to have a sense of inches, you could just stand the paper vertically, for a 
moment, to get an idea of the height you will have to lift the phone. Notice 
the position of your arms/elbows and try to aim for that.
