I know TiVo comes up from time to time as an accessible DVR option. I've written a fairly detailed review of it from the perspective of a blind user using the iOS app which can be found at http://www.blindtech.net/?p=33 The short summary is setup is not accessible but if you have the disposable income and an iOS device the TiVo is worth buying if you want an accessible DVR solution.
