the milter protocol requires mail to be passed as received - without
locally
added data (including local Received: header).

. However, SA
considers internal only headers above the first Received. So, the
spamassassin as a milter architecture makes passing internal headers to
SA impossible.

milter protocol supports passing much more data than the mail body to
milters. The problem is that SA only supports mail body and nothing
else, to
all other data must be passed through pseudo-headers. Which makes it quite
hard when you need locally added Received: header after data received from
other milter.

"SA only supports mail body and nothing else" is incorrect.
SA expects the pristine message.
It can only process what it sees.
