Q1. Does "Download Manager" really appear when you save an email
attachment - I thought it was only for http downloads from web sites?

Not normally, at least I've never seen it in that context, and I've been here since Netscape 4.7. But I haven't changed my prefs lately, so your friend's prefs may differ from mine.

Q2. Best way to turn it off completely?

Try this:

Edit | Preferences | Browser | Downloads
(o) Don't open anything
(•) Open a progress dialog
(o) Open the download manager

or

(•) Don't open anything
(o) Open a progress dialog
(o) Open the download manager
