What about not messing with qmail-queue or qmail-scanner at all.
Just use a .qmail file in the user's dir.

$ cat .qmail
| /usr/bin/spamc |/usr/local/bin/maildir ./Maildir/

This seems like a good clean way to implement SA to me.  Users that want
SA can have the above in their .qmail file, those that don't, don't.

No patching, has worked for me (for an admittedly smallish system) for
over a year.
