qmail is "patch city"
sendmail is "upgrade city" (this year) :-) 

With qmail, either use qmail-scanner or replace qmail-smtp with qpsmtpd

With qpsmtpd you can find some plugin to call SPAMD. 
This plugin is maybe not "rock solid" but you can tweak it
(it is just some perl code).
