Thanks, that led me to the solution.

 - It turns out that this header is not from my local SA install, but
   from a mailing list I'm subscribed to (which was the original recipient 
   of the spam.
 - I re-checked the headers that do correspond to the local server, and
   found that SA was not being run at all!!
 - It turns out that I must have mistakenly deleted my .forward and
   .procmailrc files and THAT is why my MTA stopped calling SA.

I don't know how I did this.  I don't think I would type "rm .forward 
.procmailrc" without noticing.

But well, problem solved.  Thanks for the help.
