John, in your local.cf file put the following:

rewrite_subject      1
subject_tag       PROBABLE SPAM: (or write in whatever text
you want)

I use the following instruction:

subject_tag     [Spam Score: _HITS_]

What this does is give me a subject that looks like this:

Subject: [Spam Score: 25.50] Want to Create your own DVD library?9 237948

For me, it is useful to see the score, simply because that
is a good indicator of likelihood of spam. But you
can substitute any text you want.

If what you are looking for is a way to REPLACE the original
subject rather than just add a prefix (assuming you find the
subject offensive), I don't think that can be done, but you
might experiment with creating a very long tag, like
"PROBABLE SPAM........................................" -
presumeably that would push the offensive stuff outside the
viewable area of most email programs.
