John,

Upon further investigation, it appears the author of qmail-scanner has this
to say about vpopmail:

[snip]

Q-S doesn't work with Vpopmail Vpopmail basically strips out environment
variables set within the tcpserver SMTP rules file - specifically the
QMAILQUEUE environment variable. As it is responsible for starting
qmail-smtpd, that means Qmail-Scanner never gets called. This is really a
bug with Vpopmail, but a workaround is to set QMAILQUEUE within
/service/smtpd/run instead. However, you must realise that you will lose Q-S
functionality - such as altering Q-S components based on SMTP server IP
address, etc. This will only get worse...

Which is a bit unusual as I'm running vpopmail without any issues
whatsoever. However, I should also mention that I'm using a much, much
older release of vpopmail that has been modified slightly to support
PostgreSQL in the manner I desired. To give you an idea just how old, it's
version 4.10.32...

If I have an opportunity to tinker later today, I may download the latest
tarball and install it on my test setup just to see what happens...
