If the rules were compiled with tcprules, you should be good to go (on the
other hand, you probably wouldn't be able to relay through the server if you
didn't...).  Aside from forgetting to compile the rules (I've done this a
few times and then slapped myself for it), the only other thing is
forgetting to restart qmail after replacing the qmail-smtpd binaries with
the ones that were patched to include QMAILQUEUE...

The next thing that comes to mind is to have a glance through the
qmail-scanner FAQ:

And just double-check that it's setup correctly.  The options it recommends
for SpamAssassin's spamd are fairly dated, however.  Also, you could also
have a look at qmail-scanner-queue.pl and verify that it sees SpamAssassin
(just search for spamc and if you get a match, it found it).

Worst-case, I suppose you could try setting the localhost QMAILQUEUE (in the
tcprules) to qmail-scanner-queue.pl -- although that really shouldn't
matter...

I'm curious.  Are you running qmail under daemontools?  Also, if you look at
your netstat output, do you see spamd running on port 783?  Does it show
anything in the maillog output?  (This should show up with spamd in one or
more of the entries and may provide some clues as to what's going on.  Try
sending an e-mail while tail'ing the log and see what happens.)
