I think that in headers the =CHARSET=?...?= syntax must be used since Content-Type: doesn't apply to headers. I used to see this problem with only some messages. Perhaps the foreign characters do cause SA to get confused, but I never actually looked for a pattern. After setting OPTIONS="-F 0" to /etc/default/spamassassin I haven't seen the problem since.
