Personally, I use a VPN connection through the firewall to manage any
systems inside it. The details will depend on which firewall you have of
course.
The other option is opening up ports on the firewall, which I would avoid if
at all possible.

I have attached a version of the VNC server with the
tray icon disabled. To answer your first question, if
the tray icon is not present then the users will not
be aware that VNC is running and will not have access
