I had the same issue, also with 5.11.0-44-generic, reverting to kernel
5.11.0-41-generic fixes it. Prior to this I upgraded from Ubuntu 21.04
to 21.10 and no USB port worked, with kernel 5.13.0-23-generic.

After much cursing, I'm slowly making progress while trying to install
this sucker. Removing everything in the /etc/vmware dir seemed to just
mess things up for Adept. Plus, it didn't work here, so I don't
recommend it.

Forgot to mention download alternative cd install in text mode
when you restart choose safe mode option.Wait for command prompt type lspci
get the busid number for your pci graphics card mine was 01:00:0 it might list
two use the vga one.Then back at the command prompt type sudo dpkg-reconfigure
to fix this issue start in safe mode and wait for command prompt may take
awhile then do "lspci" look for ati vga.Then write down pci bus number mine was
1:00:0 restart again in safe mode and wait for the command promt.Then do sudo
dpkg-reconfigure xserver-xorg you will then see options to set u
I have a similar error in VMWare making it say "unrecoverable error
(vcpu-0): NOT_REACHED F(562):1742"
