I have had a look at the SpamThrottle patch (http://spamthrottle.qmail.ca)
and it looks like the SpamThrottle mechanism does not take into account
SMTP AUTH, can you confirm whether this is the case or not.

already have SPF in Spamassassin which included in QmailScanner. To add it
in the Toaster would be an unnecessary duplication. Plus, it seems more

email to hotmail.com it accepts it but does not deliver it.

accepts the email for delivery but the email is
never delivered to the hotmail account.

Check out junk folder? Sure that your server is not in blocklists?
