For quite a while now, many many months, I have had the problem with my iPhone
eight, always running the latest iOS, wherein voiceover would just shut up. All
the sound effects would still be there but there would be no voice. My solution
has been to turn voiceover off and then back on again
Yes, not that often - and I’ve never detected a pattern to it either.

Yes, I have experienced this, but not as persistently as you have, Phil. My
solution is the same as yours. Turn voiceover off and on. But I only have to do
it once, and it doesn’t happen every day. This is both with an iPhone 7 Plus
and an iPhone XR, so it is not limited to just iPhone 8.

Yes, I experience that problem quite a lot! I have an iPhone 7, and I am also
running the latest iOS, 12.4. I thought it was something I was doing wrong, LOL!

It seems like when i try to use Vm fusion 11 which i apparently upgraded to i
no longer have usb devices working.
THey seem just gone and i have tried all i can think of.
Remove the usb controller and reinstall it.
I have reinstalled vmware tools.
No go.
Any thoughts.

What you need to do is to go to Apple > System Preferences > Privacy and
Security. Once there, select the Privacy tab, and make sure that Fusion is on
the list of apps that are allowed to access data on your machine.

To add to Simon's info, you get to the Finder using cmd-tab to cycle through
apps, or select Finder when on the Dock. Also, when in the Finder, you could
press cmd-shift-i to open iCloud, locate the Desktop folder there, open it, and
paste into that item with cmd-v.
