I've not had problems with this. Normally, like Nancy, I navigate to the
bottom of the message to find the attachment. From there, I just VO-space on
the text of the attachment and the menu comes up whereby I could open, save etc.

This would not be a VoiceOver issue. It likely has something to do with your
settings within the Mail Preferences whereby it's keeping a copy of the draft
both on the local machine and on the gMail servers.

This is a Google issue. There is an option in Apple Mail to compose drafts
in a local folder. This will help solve this issue, but even when I compose
a message using gmail's web interface I get lots of drafts.

Your question is difficult to answer because OverDrive for iOS, while VoiceOver
accessible, is not the most intuitive of iOS apps to use.
