Problems like this where the header or footer is missing in the printed output, are almost always due to the fact that LilyPond by default gives A4 sized layout and you use letter sized paper in the printer. Apparently, this was exactly the problem in this case, see http://lists.gnu.org/archive/ht

One possible solution is to use a separate Staff context, where you have peeled off most normal engravers and reduced the font size. There is a ps2png program included in the LilyPond source code, so it's probably a packaging error that it's not included in the debian package. However, if there's a risk for name collisions with other packages, it's maybe not a good idea that the script is called exactly ps2png. /Mats Karl Ham
