I just ran a series of tests with a hebrew document and lilypondbook.
I finally got it to work for me by surrounding all lilypond additions
with \l {}
So now there is a way to do both lilypond and lilypond-book in hebrew.
However when I tried this with earlier versions of lilypondbook th
(I'm afraid you have seen this answer before, but that's the best
anwer I can give.)
You need to find out a way to write an ordinary LaTeX document
that mixes Hebrew and latin text and gets the typesetting in the
correct direction for both the Hebrew and the latin.

After reading the template died thread, it dawned on me that I have been
having a similar problem with lilypond book.
The story goes like this.
I create a hebrew latex document. It looks great...
I add lilypond examples one of two ways:
1. a complete hebrew lilypond file and it chokes
The answer all depends on what changes you want to make.

For example, to get a larger font for the title, you could do

The lilypond script has hard-coded the distance from the bottom of the
score to the baseline of the footer text to be 2 mm. This seems silly
to me, since the bottom margin of the page (including the footer text)
is set to 20 mm. I think this is a clear bug which should be fixed
as soon as possible.
While I am at it,
I get lilypond to print hebrew using a hack of the lilypond and
lilypond-book file cmd=elatex.
Is this still my only choice?
Also I don't remember what these scheme functions do.
I removed the stuff for alignment since I don't need them anyways.
