I have just finished to build kde4.2.3 with qt4-4.5.1 on amd64.
Except the already noticed phonon/qt4-phonon switch, everything else gone right.
Until now I got only an issue with menu's launcher which shows nothing
when clicked.
Let me know if I must perform such test.

Same issues, same workarounds here on
8.0-current amd64
-lsmokeqt
Starting from an old job of Max, after the installation of
kdebindings-smoke-qt, now your kdebinding4 builds and
system-printer-config-kde runs.

While dolphin is loading kill X with ctrl+alt+backspace then restart
kde4. Now it works.
Some process (kded4, mysqld, pulseaudio and an unknown 'sh') survives
making the second launch the right start.
I'll continue to investigate.

tem kde4.2 has some problem talking with dbus.
Running 'dbus-launch konqueror (or dolhpin)' within konsole improves speed?

I got some trouble re-building kde4.
Adding phonon between kdelibs4 and kdebase4 QT_COMPONENTS should fix this issue.

A component listed in bsd.kde.mk (qt4-inputformats) doesn't exist in
port's tree.
A whole s/inputformats/inputmethods solves this issue.
Many thanks
