I don't know the internal details for FreeSurfer, maybe others can help with
those. But in general, the vertices and faces are constructed *together* from
the (pre-processed) image voxels by algorithms like Marching Cubes:

The distance between vertices differs. Roughly speaking, areas with higher
curvature show higher vertex density.
As for your first question, I am not sure what exactly you mean, could you
explain in more detail what you want to know?
