Ok, after some more testing with new GNOME Shell (3.32.0 + some commits
backported), the situation has now improved when this case happens:
- there is no more icons for applications not having any appinfo, like the
update-notifier one.
- applications with appinfo have an icon with a decent size (
@ian-gordon does this mean the fix does not work for you? I'll release
it into bionic-updates since it seems to work for others and not
introduce any regressions, but if you are still encountering issues

Tested all combinations of both (Ctrl-s/context menu in safe mode and
normal mode), all with the same results. Could this be locale-dependent?
The UI is in English in safe mode though, so I'm guessing safe mode
should be locale-independent, as I'm on fi_FI.UTF-8 otherwise.

(Firefox and Thunderbird)

Sometimes when resuming from suspend pulseaudio is broken and only the
dummy output is available. After "pulseaudio -k" audio again works.

bee fixed mainstream and the fix is available in other distro, so if any help
