Ok, after some more testing with new GNOME Shell (3.32.0 + some commits 
backported), the situation has now improved when this case happens:
- there is no more icons for applications not having any appinfo, like the 
update-notifier one.
- applications with appinfo have an icon with a decent size (typically 48x48).

I think if we really want an application to have the icon showing up,
let's add then appinfo to it, otherwise, the current behaviour is fine.

Bug description:
  I got a "update-notifier is ready" notification, with a HUGE icon
  which made the notification huge.  update-notifier is a service, so
  shouldn't be opening a window itself.  After the notification I never
  actually got a window.  Also the shell should scale the icon to a
  proper size.
