the /usr/X11R6/lib/X11/app-defaults you can send a bug report for it.
It needs to be recompiled against X4 to move app-defaults to /etc/X11.

Most likely answer, you still have woody in your sources.list. Due to a
recent reorganization of the ftp archive woody==testing and
sid==unstable. In order to get the X4 stuff from official debs you
currently need to get them from sid<-->unstable.

Not that this is the answer, but I am running Sid here, X 4.0.2 libc
2.2.1, Nvidia card as noted above, stock X drivers 2.2.19pre kernel, and
no problems with X here. Are you guys maybe trying to get the Nvidia
drivers installed, using 2.4 kernels? DRI patches?

Check bison from today 27 Feb 01, changelog mentions bison.simple
missing from a previous upload.

Try for your header that most X apps will need. You'll likely
need to add the following to your $(CC) line:
-I/usr/X11R6/include
and this to your LDFLAGS and/or $(LINK) line:
-L/usr/X11R6/lib/
