I've just tried setting up the radosgw on centos6 according to
http://ceph.com/docs/master/radosgw/config/
There didn't seem to be an init script in the rpm I installed, so I
copied the one from here:

Yes I upgraded the cluster, it is definately not new. I ran into
several other issues as well, for instance legacy tunables that had to
be changed.
These are the directory contents you asked for:

It would also help if you could add
ceph tell mon.'*' injectargs '--debug-mon 20'
check that it is set (replace mon.a with mon.)
ceph daemon mon.a config get debug_mon
{ "debug_mon": "20\/20"}
and check what the log have after you get
ceph osd pool create test123 100

Did you upgrade the cluster or is it a new cluster ? Could you please ls -l
