I know that the post is old, but after some head banging I found the clue ...
I had the same problem with two LTO4 tapes (I own an Overland Arcvault 24 with
a server running ubuntu 14). The server crashed before bacula be able to end
the tape writing for these two tapes.

It was happened with tape which was in the drive and written in time of
server reset. There are two tapes with such problem, because server was
reseted two times. There are two new tapes, writing first time. I think the
problem is the end-of-data lack on this tapes, because server was hard
reseted
Is this happening with this one tape only, or all tapes you try in this
particulat drive?
If the problem is only with this tape, is the tape old and been used a

I run my bacula on FreeBSD 8.0-RELEASE (ProLiant ML370 G3) with HP MSL6000
ULTRIUM960. My bacula server was hard reseted then bacula write on tape.

I previously posted this issue on this list but there seems to be no
solution. I just upgraded to the latest release of bacula and the
