I have the same problem both with 8.04 and 8.10 (waiting for Jaunty...), 
when using everything that makes sound, from rhyhtmbox and vlc to warcraft and 
firefox.

I have an audigy LS CA0106 (witch i`m using as primary sound card) and an 
onboard.
Have searched the forums for any problem ever bothered me (like pulse audio 
forgeting the default sound card) and eventually found solutions that worked 
for me, EXCEPT FOR THIS ONE.
Have all the volume meters MAXED (those with and without GUI).

On windows I use to set my sound to 3% for everyday use.
On ubuntu I have to set it to more than 30% (more likely something between 
40-50%).
Seems like a driver problem to me.

Hope a solution will be found sometime.

It looks like the problem is with the SATA controller. The relevant line from 
lspci:
00:1f.2 SATA controller: Intel Corporation ICH9M/M-E SATA AHCI Controller (rev 
03)

I have found the exact same problem in Redhat bugzilla #476392:
"  Suspend and Hibernate don't work on HP DV5T  "

The HP DV5T has the exact same SATA controller, the symptom are the same
too.

What is the proper method to notify upstream? Looks like the bug is
identified.

Now I'm using Tilda on Archlinux with E17 window manager. And Tilda is
working...

Don't know if it really helped, bet when Tilda is always crashed at start up, in
configuration I added „Run a custom command instead of the shell“: „zsh“.

After this Tilda started to work. But that was long time ago.

And now, when I run Tilda from terminal I get similar warnings:

I wanted to add to this comment. This problem only happens when you
upgrade Calc from 2.4 but if you do a fresh install with the same repo
the highlight color is the nice blue that is very noticeable. I use this
repo to get openoffice:

deb http://ppa.launchpad.net/openoffice-pkgs/ubuntu intrepid main

I noticed today that the highlighting was light so I did

sudo apt-get remove openoffice*

then I went back and installed it again and it came up right.

Hopefully this makes it easier to solve. Thanks!

This situation can occur when dolphin decides to show a right scrollbar,
this leads to another flow of icons, then scrollbar is turned off, then
on, then off 

Workaround: Make window wider or narrower for a clear decision to show
scrollbars or not. I have newer seen this behavior of dolphin before. I
am on a freshly updated jaunty system on amd64.

1) Please get Grammar Check in OpenOffice 3.0.1 (Ubuntu).

2) "Highlighting" 
To undo Highlight, one has to always select "No fill" by clicking on the 'drop 
down' arrow of the 'Highlighting' box. Whereas, highlight after selecting a 
particular color I just have to click the "Highlighting" icon. (Without 
reproducing the two steps of accessing the 'box' every time. Please let me know 
if I am not clear on this issue. 

3) Caps Lock change An automatic inversion is taking place. This has
already been reported as a subsequent comment of  Bug #227326. I have
subscribed to that bug and will follow it up there.

Finally I replaced my Hitachi/LG GSA-T50N with a Samsung SN-S083B drive.
The bezel is compatible so it sums up to removing two screws and paying
EUR 28,00.

After the drive installation I upgraded its firmware to the newest
version (SB01) that can be downloaded from http://www.samsungodd.com/.

The drive works perfectly and passes all tests that the Hitachi/LG model
failed.

It is disappointing that Asus Germany is unwilling to recognize the
severity of this problem. A simple download could have fixed this issue
faster and cheaper.

I also find it strange that the drive manufacturer Hitachi/LG does not
offer any support for its drive although it is obviously not working
correctly.

My setup:

Dual core X2, 2gb ram, sata hard disk (sdaX), etc.

A windows vista partition (came with laptop -rarely- used... 'when I have to')
A normal boot partition
An LVM partition

I am going to postulate where the problem originates by explaining an earlier 
statement I made.

I have three ext3 partitions with internal logging, all of which live
inside of encrypted LVM containers.  I am unsure where in the chain the
problem occurs, but this is what I think may be happening.

1) Something modifies the state of the disk, logging info/etc.
2) That change constitutes a change event.
3) Change events are bad, but even worse, may be logged, feeding back in to 
step 1.

A recursive notification loop would thus form.

In conclusion, to quote a line I said in IRC "what lesson did this
illustrate: Never inotify for normal operations of a special device
file..."

Thanks to both of you for theses patches. I merged Siegfied branch since it's 
the more up to date and I was also willing to remove the debian directory.
I don't think webboard-applet needs a man page. You won't normally execute it 
since applet can't add themselves to the panel (you are forced to use the panel 
menu to add them).

I recently noticed that this problem was caused by Pulseaudio.
After killing pulse audio process CPU usage was back to normal.

It looks like Pulseaudio was trying to play music through the video port...
My card has HDMI port which might use this feature, but I have my monitor 
connected to d-sub.


More over after installing latest ATI 9.1 drivers the issue seems to be gone, 
but can't confirm it, since computer goes into freeze after 2 or 3 minutes. 
(terrible, I can't use it at all, will try to go back to previous 8.12)
These freeze occured on 2 machines I tested: ATI HD3450 and Radeon 9550.

No, it was not an issue for me when I submitted, as the bug also 
includes the underlaying reason for the bug and my resolution.

This bug is about the Hardy -> Intrepid upgrade, which is still the 
latest release to my knowledge. I was able to rescue my broken system, 
but other users might not be, hence the submitted bug.

You can close the bug for all I care, but I think that an unbootable 
system after the upgrade is a severe bug.

Yes. Occasionally I have, possibly shortly before the message came on-
screen.  I think this failure happened occasionally prior to the jaunty
upgrade too.

While coming back.  One is left with a black screen with a cursor
flashing in the top-left corner.

I'll try it out soon.  From memory, over 10 cycles it probably does fail
once,  but I need to check.

Yes it works most of the time.  But now and then it fails.

this failure, for example did your battery run out? Did the machine lock
up, did any messages come out? If it is reproducible could you try the
suspend from VT1, press ctrl-alt-F1, login, and run pm-suspend there. If
you could test and report back here that would be helpful.

I'll hopefully do this in the next couple of days.

The TJ "fix" did NOT fix anything for me. Same symptoms as most here:

Appears not to boot (within a reasonable time-frame) with 2.6.28-8,
spewing out dozens of messages of the form:

/sys/devices/pci:00/:00:1f.1/host0/target0:0:0/0:0:0:0/block/sda/sda2
(9365)

to the console every five minutes or so.

2.6.28-7 does boot (slowly), lots of disk activity. udevd then takes up
c. 50% of the CPU.

Disks are PATA (mobo i/f) and SATA (addon PCI card). In each case, LVM
on top of RAID1.

Me-too for the high CPU usage by udev 138-1. Downgrading udev to 137-1
and dmsetup to 1.02.27-3ubuntu2 there is normal CPU usage again. I have
ext3 root filesystem on an LVM logical volume, home is mounted as
eCryptFS.

I can't reproduce by following the exact instructions given here, but
see the exact same symptoms by following these steps:

Way to reproduce:
1. Make sure Metacity is the window-manager (ie. disable desktop effects)
2. Open firefox
3. Reduce the window size to cover only a part of the desktop.
4. Open up another window, for example your home folder, and place it on part 
of the screen that is currently not covered by firefox
5. Maximize firefox
6. Restart firefox
7. Unmaximize firefox. It will return to the size decided in 3).
8. Click the other window opened in 4) to switch focus to it.
9. Click the firefox window. Focus returns to firefox, but in the same time it 
changes the "unmaximize size" to cover the whole screen. The size from 3) is 
now forgotten.

This is on Ubuntu 8.10, metacity 2.24.0, Firefox 3.0.6

For what it is worth, it seems that it works as it should on Debian
Lenny with metacity 2.22.0, Iceweasel 3.0.6. When I try there Iceweasel
does not resize at step 9, and does not forget its "unmaximize size".

Except this calender and volume issues.

In my point of view, this is a small piece of compiz settings configuration 
work.
And does make the whole desktop all of a piece.

If you read my error report more closely you can see that:

1) filling of the root partition is not the cause
2) it's a symptom

My root partition has plenty of empty space (used 4GB out of 26GB) and
the problem starts occurring when it suddenly reports 27GB of 26GB is
used. This isn't related to any programs filling the root partition. It
can occur at any time randomly regardless of what programs i'm running
and with only with precisely one kernel version.

Anyway, it's good that your message raised my attention on this report,
because the bug seems to be gone in the current -11 kernel version. It
was only existing in -9 and not in the previous -7.

According to the Intel BIOS manual there seems to be a setting in BIOS
that let's you to choose your SATA mode (Advanced > Drive Configuration
Menu > Configure SATA as) which is by default set to "IDE". Do you have
tried "AHCI" instead?

jh, can you get in or chroot and run:
sudo dpkg -l udev

You should see this:

rule-based device node and kernel event manager

If you do, please run this: (I use -c since -u has broken the cryptsetup in the 
past.)
sudo update-initramfs -c -k all

If you do not, consider adding TJ's ppa OR following the removal of
watch instructions I posted in an earlier comment.

I would like to add information that may shed light on this.
I too started getting this warning in the log after the upgrade from hardy to 
intrepid

could it have something to do with changing the time on the system?

For example, if I run hwclock --systohc + adjtimexconfig, I see the
warning after a few seconds in the log. Here's the sequence:

This makes me suspect that changing the hardware clock, causes the hda-intel 
driver to wrongly conclude that something might be wrong and activate the 
presumably inferior "IRQ timing workaround". A pity.

I had this strange behaviour too.

On a MB (GA/nforce560) I used a Seagate 7200.11 (SATA) and a fresh install of 
Ubuntu 8.10.
I noticed a HDD activity every 4-5 sec, which was very annoying.
Because of the Firmware bug of the Harddisc I decided to replace the HDD with 
another one.
I took a Seagate 7200.10 (IDE) and never noticed this problem again.
-> so the problem seems to be caused by something SATA-related (?)

But it became more crazy: After flashing the firmware of the 7200.11 I put it 
into another
PC (GA/nforce3-150) and also did a fresh install of 8.10. The problem did not 
occur on that system!

So in  my thinking the cause of this problem is:
- either the firmware of the hdd (cache flush?)
- the driver or some driver related software of the nforce560 sata controller

I hope this can help you locating this effect.

When multiple users are logged in in multiple GDM session, an inserted
cd/dvd will be mounted by the user that was first logged, without
regards to the session in which the cdrom is mounted.

This makes impossible to unmount the device under the last session (an
error dialog shows up), one must change to the first user in order to
correctly unmont and eject it, or force it with a sudo.

A correct behaviour would be that the device is mounted by the user that
is currently active.

What is different when you run it as a user and at startup? In both  
cases the nvidia drivers aren't loaded yet? You are certain that you  
use the same binary in both cases?

Perhaps it failed to report bios info when running from a startup  
script. Could you check if the output of 'nvclock -i' shows bios info  
in both situations?

The attached slide started life as a powerpoint in OO, then it was
edited in powerpoint and sent back to me. The tableis totally corrupted.

Same happens if I copy a "good" table from powerpoint and paste in OO
(ppt format). Also first time I saved this file OO crashed

I think there is a major issue with Tables in both OO Writer and
Impress. Pity cos apart from that I find OO quite useable however I use
a lot of tables...

And another thing, I remembered, could help. ACPI (I mean battery status
and some tempeature sensors). After system start, it works fine, but
after a while (say 10 minutes?), it just stop working, sensors are
showing 146 degrees of C, battery = 0% - not present, etc. I tried to
suspend just after system booted up, to be sure, that ACPI is working so
far, but still fails. And no, unfortunately, I can not see any messages,
when ACPI stops working :-(

Unfortunately I can not reproduce this anymore, since I changed my
livingroom TV. With this one I have another problem - only allows
1280x720 (or something like that), although this is a Full-HD TV
(Samsung LE46A656). Anyway, that's probably another bug. That is with
9.04. Before upgrading I also tried with 8.10, with similar result - I
only saw modes up to 1366x(something), but 1920x1080 was not there.

when the installer could not configure my network.  Once I got to the
desktop I found that Ubuntu was not even seeing a NIC.  I rebooted into
Vista x64 to look for solutions only to find that my device manager
there also found no NIC.  I checked the BIOS settings but there was
nothing wrong, the onboard NIC was still "enabled."  The only way I was
able to activate the NIC was to reset my CMOS back to default settings.
Even then Ubuntu would not connect to my network.  Windows was able to
run properly though, so I tried installing Ubuntu again.  After the
install I found that my network card was once again disabled, once again
requiring a BIOS reset.
