Hi Fariborz,
 
We are faxing both Radiology reports and lab results with a system we built inhouse using Linux.  We have a Fedora Core 4 server with sendfax installed.  We wrote a Python script that receives a print job from Meditech and sends it to the fax spooler.  The Meditech system simply sees the faxserver as a printer.  It's been working great for us.

Assuming that you are accurately changing the AWP as they come on the
invoice, there have been SIGNIFICANT reductions in AWP over the last 5
years.  Some companies have gotten in trouble for "inflating" the AWP so
they are getting more realistic.  If you have FirstDataBank, make sure
you are using the BLUEBOOK value from them as it is the AWP we are all
used to.   However, they are going to stop reporting this in the next
year or so.  We have to constantly monitor any reductions in AWP to
possibly adjust our charge formulas.  I download the data before
updating drugs by FSV then comparing after. HTH.

Hello experts,

 

Because I am the analyst for Pharmacy I am now being asked to justify
why Pharmacy Revenue is dropping.  Have you any of you had to explain
this?

If so what did you determine. Several of us are working on this and
cannot determine why revenue is dropping from year to year.  We charge
by AWP markup.

Several drugs are going generic so therefore the AWP is less and the
patient charge is less.

 

Have any of you found solutions that pertain to how the computer is
charging? What are some other possible solutions?

We are C/S 5.5 SR2 and we use the Distribution Lists in the
OR module.
Once the surgery schedule is finalized in the afternoon, the
surgery scheduler force prints the next day's schedule from
within the GRID to the Locations/printers defined in the
appropriate Distribution Groups.

[MEDITECH-L] BMV System

You can default previously filed responses into a multiple line query with the 
following attribute:

- Replace the QUERY with the desired query mnemonic whose responses are to

- Change the two >2’s in the last two lines to the number of lines in your 
query.
- The two 25’s in the last two lines should be changed to the length of your 
query.
- You may replace the 1 at the end with “” to make the response default
and not be editable.

I have not not tried this on an E type query.  I think it will work but I'm not 
certain!  I know that the F5 recall key does NOT work in an E type query but I 
do have a macro that DOES default previously filed responses in E type queries 
and is pulling from ADM.PAT so I think this will work for those as well!

We have EDM/POM med ordering and BMV live.  Here are notes I used for
our pharmacists training sessions and 'where we put what', including
Label Comments.  Meditech doesn't really give you any guidance in
deciding where to put information.  You just need to look at the various
fields from upstream and downstream with the POM and e-MAR products and
try to decide what each field is best suited for in handling order
information.
 
Dose - we will be trying to eliminate zero-dosing as much as possible.
Items like insulin, MDP oral liquids, compounds where the dose is in mgs
(e.g. spironolactone suspension) will be entered with the numeric value
instead of zero-dosing the order.  The MDP order types are now
suppressed from refill lists by the order type, so we no longer have to
use zero-dose to keep scheduled doses from dispensing.
 
Dose Instructions - still need to use this field for some multi-dose
items because the dose is non-numeric.  Examples:
Apply liberally
Sparingly
Thin layer
Remember, only dose goes here...nothing about
site of application
 
PRN Reason - Currently not required field, but probably heading that
way.  This field accepts free text or dictionary entries.  We have to
manually build each PRN Reason and attach them to the appropriate drug.
If we don't have anything attached to the drug, ALL PRN Reasons show in
the lookup.  This can be too many to look through.  Once we attach
something, then it is restricted and easier to identify the appropriate
reasons.   Examples:
SBP greater than 180
PRN sleep
Nausea/Vomiting
Constipation
As Laxative of Choice.
Agitation
Muscle Cramps
Itching during dialysis
After each loose stool
Mild (score 2-4) pain
If you have a PRN Reason that is too long to type into this field, then
it belongs in Admin Criteria.  


Label Comments - This is now used for pharmacy ancillary
comments/generic drug information or instructions to nursing.  Examples:
Note Dose
This drug entered with multiple routes
Shake Well
Refrigerate
Therapy beyond five days not recommended.
Therapeutic Interchange messages
NF drug entries (generic, dose, brand name)
Give 30-60 minutes prior to meal time, unless ordered
otherwise
Extended Release Table - Do Not Crush
HYDROCHLOROTHIAZIDE 25MG/TRIAMTERENE 37.5MG

Prep Instructions - For Pharmacy, Nursing, Respiratory Therapy special
preparation instructions.  If there are prep instructions, they will
print out on a separate IV label and can be useful for the IV room.
Examples:
   Pharmacy:  To prepare D12.5W, withdraw 31.25 ml
   of fluid from a 500 ml bag of D10W.
   Put 31.25 ml of D50W into the bag. The
   total volume will be 500 ml.
 
 Nursing: Add 2.5 ml Sterile water to 250 mg vial
 to make 100 mg/ml concentration and
 withdraw ordered dose.
 
Special Instructions - First line used by pharmacy for special tubing
instructions.  Otherwise, nursing can use this to communicate between
shifts for special instructions to one another.  Examples would be,
patient won't take with cranberry juice.  Remember, having something in
the first line will cause a second label to print out that will be
attached to the product to help ensure it gets tubed to a special
location
 
Admin Criteria - This is where all physician ordering information
belongs.  It can be one of three formats (text, table, CDS - customer
defined screen).  Text is the most common when you are free text typing
in information.  The description field is required, but doesn't show
anywhere (I just type 'ft' for free text).  The 'Display' field will
show on the POV if you enter something there, otherwise just type in the
text box.  Table structure can be used, but it is probably more for
order set type order such as Heparin Protocol, Sliding Scale Insulins.
CDS's have to be built by PHA/IS.  Examples are PCA settings, TPN NPO
Rate.  Examples of Admin Criteria
Left eye
Both eyes
x1 dose
After four (4)pm day before surgery.
Apply to both Nares while CPAP ordered.
Discontinue EPO post-operatively when the Hgb reaches
13 g/dl. If there are signs and symptoms of
bleeding, EPO
should be continued until bleeding is ruled out.

  We have upgraded to Magic 5.61 and on many
of the orders a lower case "r" appears in the
Status field on the profile. I know that this represents
" PHA Review Activity Exists ", but I can't figure why
this is appearing. This seems to at least impact our
transfer med list (meds drop off that have the "r" as 
a status). Also, it seems that even if an order is reviewed,
the "r" may return as a background job even if no pharmacy
activity has been performed on that med.
 We only have the pharmacists do order entry at our facility.

destination. Your FTP host is replacing the dash (and probably other

I'm hoping someone can help me with an puzzling ftp issue. I have a
report scheduled to go to an ftp directory. The report runs fine and
lands appropriatly to the ftp directory at the designated date/time.
However, in this particlar case I have a file naming requirment that
includes the "-" (dash/minus sign) character, i.e. "filename-1.txt".
When the report is deposited to the ftp directory, somehow, the "-"
character gets changed to "_". I've tried a number of things, report
title of "filename-1.txt", "filename-1.txt"^/ZFTP.TITLE both from a
footnote and macro, I've tried "filename"_(1*2-1)_".txt" and other
similar efforts to get a "-1" character, but all attempts leave me with
"filename_1.txt". Does anyone have any ideas why this happens and is
there a work around. We are a Magic 5.5 site

Hi Janet,

I'm not sure what additional things that you are wanting to do... but
you can use as many FCL attributes as you want as long as you
sequentially number them like this:

FCL1=IF(@.response="Y" 1,
FCL1=%MIS.SCREEN.display.doc("MYSCREEN","QUERY")}
FCL2=IF(@.response="Y" 1,
FCL2=%MIS.SCREEN.zcus.pgm.M.display.doc(A,B,C,D)} 

Following this format, you can do as many things as you want based on
the query response of "Y".  If you have simply copied these attributes
from another source... the syntax error is a result of trying to call a
macro program that does not exist in your system (as well as the fact
that both of your FCL's were FCL1).  When copying attributes, you
usually must do some editing to pass the correct values as well as to
add any custom macro programs into NPR.  If they aren't there, you will
get the syntax error.

Doug - No idea why it's happening, but one workaround I would try is to
insert a decode into the variable:

"filename"_D(45)_"1.txt"  (45 is the ASCII code for dash).

However, in this particlar case I have a file naming requirment that
includes the "-" (dash/minus sign) character, i.e. "filename-1.txt".
When the report is deposited to the ftp directory, somehow, the "-"
character gets changed to "_". I've tried a number of things, report
title of "filename-1.txt", "filename-1.txt"^/ZFTP.TITLE both from a
footnote and macro, I've tried "filename"_(1*2-1)_".txt" and other
similar efforts to get a "-1" character, but all attempts leave me with
"filename_1.txt". Does anyone have any ideas why this happens and is
there a work around. We are a Magic 5.5 site

We have parameters set to 50% before, 50% after, and 90 Minutes.  Then we do a 
lot of individual settings for specific interventions.  

For example, if vitals are not changed to 10 Minutes out of range, if you have 
vitals scheduled every 15 minutes, documening the very first one (where the 50% 
can't be calculated by the system), the 90 minute setting will remove 1 ½ hrs 
worth of "Next Due" vital signs intervention flags!

Likewise, we changed our Before setting to a very small number on our shift 
physical assessment intervention. If left at 50% before, a nurse documenting 
the patient assessment for a new admission in the second half of the shift or a 
reassessment for an existing patient would cause the next shift's "Next 
Intervention" flag to disappear.

charges are based on AWP.  To my knowledge there is no way in the system
to determine when the AWP changes and what its values were.  We do not
load AWP from a formulary service. It is a manual change.  Please let me
know what you can share on this matter.

I've never tried this, but it strikes me that, if you set up one report as the 
"master" and all the others as fragments of that report (run in the trailer 
region), you ought to be able to get what you're looking for.  If you want to 
select from various reports, you'd have to set up a select variable(s) with an 
IG operator, and then use the selection on line checks for each fragment.

[MEDITECH-L] LAB: COBAS 6000 / MIDDLEWARE

We are a magic 5.5.2 site, starting implementation of Roche Cobas 6000 and 
Middleware.
Instruments are being delivered w/in 2 weeks.
One question from Roche for the set up process is whether we will autoverify 
through
Meditech or the Middleware. We currently do not autoverify any lab results 
(although 
working on doing so).  What are other users doing?  Any 
advantages/disadvantages to autoverifying through Meditech vs. Middleware?

Yes, it shows up frequently.  Do a search of the L and you will find a
thread from several months back.  These are configured in the
Custom-defined Parameters with several flags that will limit this
somewhat.  You need to add the REV function to the pharmacist ACCESS so
the can REView these and clear them.  There was a message about it can
slow down your system if they are not cleared.  It is irritating.

  We have upgraded to Magic 5.61 and on many of the orders a lower case
"r" appears in the Status field on the profile. I know that this
represents " PHA Review Activity Exists ", but I can't figure why this
is appearing. This seems to at least impact our transfer med list (meds
drop off that have the "r" as a status). Also, it seems that even if an
order is reviewed, the "r" may return as a background job even if no
pharmacy activity has been performed on that med.
 We only have the pharmacists do order entry at our facility.

[MEDITECH-L] FW: Medication Reconciliation and related reports updated on Meditech Website (Client Server)

Many of you have asked different questions of us regarding NPR issues
you're encountering on the Med Rec reports we've created that you
downloaded from the Meditech NPR Archives on their website.  We continue
to revise and improve our processes and the reports.  I have just added
to the website our latest, greatest versions.  I've asked Meditech to
just replace the original report which has been downloaded like 300+
times by now, but they don't seem to want to do so.  Said they would
update the report documentation to reference that we have newer versions
posted. 
So, you can find between 00513 and 00518 our most current revisions of
Med Reconciliation/ Physician Order report, Discharge Med Report, and
our new Transfer from ICU report utilized by our pharmacists, and an
Undefined Med report that allows you to see date, time, user, and med
entered as Undefined.

I'm sorry we just don't have the time to have our NPR writer  work

Problems are being reported at our facility regarding OE users selecting
the incorrect provider from the provider dictionary with the impact
being results/reports/consults being forwarded to wrong providers. The
issue is linked to providers with same last names and providers who
would have had the same mneumonic so one is slightly altered.

 

Our dictionary is setup  as:

We would appreciate any suggestions for either the provider dictionary,
flags to users, any change in how providers are selected, etc, etc. How
are others facilities ensuring correct providers are always selected?

We have the GE QS system receiving adt's from Meditech (Magic). What we
did is separate the QS to the Bio-med network, put a routter in place to
protect the system and allowed adt's through and connection back to
Meditech. The QS workstations have the Meditech Icon and the Nurse can
connect to Meditech and have the QS system available. We are still
looking to get a reverse feed from QS back to Meditech.

have GE Centricity EMR in our clinics.  So we are looking at the GE
product since it would be compatible with our clinic EMR and offer web
access.  

Wish I had some input but our Trace Vue is stand alone at this point.
We are hoping next year to make a decision and then move in your same
direction.

[MEDITECH-L] UB04 Type Of Bill

I can't get the Type Of Bill to print in box 4 of the UB04.  The TOB is now a 4 
character
field and the four characters (eg 0123) are visible in the spool file but when 
it prints on
the claim form, it only shows three (eg 012) with the right most digit 
truncated.  Does
anyone have a solution for this?  Thank you.

We have the Magic version of Picis' Medsurg product.  It has met all
of our actual needs.  It has fallen short of some perceived needs.
For example, there is a perception that the Block Scheduling Utilization
reports are not accurate.  Actually, they are very accurate if you
set up your dictionaries correctly.  However, our understaffed and 
under trained implementation has translated into a negative perception.
 
Our next biggest perception problem is the Magic character based 
user interface.  People really believe that we are running a PC-DOS
application based on the interface.  Of course, the same goes for
all our Meditech applications.
 
Picis' help desk has been responsive.  The documentation is outdated,
and that is a real issue.  However, it is a small one as the changes
are not great.
 
I should say that the reporting in general would not meet our needs if
we hadn't insisted on getting MSM's data repository feeds as a condition
of sale.  I highly recommend that condition if you choose this product.

We are evaluating Operating Room Management applications in anticipation of
rolling out a system with the opening of our new unit in early 2008.  We are
a Meditech Magic facility running v5.5sr3.  We have taken a look at Array,
Picis, and Meditech.  I am looking for any information from folks willing to
share their experiences that have been down this road.  We continue to
wrestle with some of the look and feel issues related to Meditech.  Weighing
the value of integration and a typically low cost of ownership of the
Meditech app against an app with greater functionality and a more up to date
user interface is not an easy task.  We need to be careful in our decision
making process to be objective basing a decision on application
functionality and economics involving staff to assure buy in for a
successful roll out.

All I know is that in similar cases, if I re-code the problem report as
a brand new report, from scratch, then it works OK...Whatever hidden
left-over inefficient code gets solved if you code it from scratch.
Might be a lot of work, but it does seem to take care of it.

We have the most bizarre issue since 5.5 update.

We have two reports (both call the exact same fragment), both supply the
exact same info for the select (verified by %Z.ddc)

I run one report - the fragment runs fast (5 seconds) (this report is
much more complex than the 2nd) I run the other report - easily a minute

I can even hard code the fragment with the select (passing @urn from
ADM) and run it by itself. Runs about 5 seconds.
I use the "slow" report to run the hard coded fragment, it hits the
fragment right away, but takes well over a minute to process it (even
though the select is already coded).

Other FYI: The fragment is pulling the patient's allergies using
meditech's mar allergy routine:
%PHA.MAR.allergies(patient,70,"",1,"",2)


I called Meditech's NPR and since have opened a task.  They said the
slowness has been reported before but didn't have a solution.

Has anyone else seen this issue?  Any suggestions would be greatly
appreciated. I just can't fathom why the same fragment (with hard coded
selects) would run so much slower when called from one report versus
another.

[MEDITECH-L] select different drug from PHA rule

The user wants a PHA rule to evaluate at MED and list drugs with the
same generic ID as the one the user entered.  I have been able to do
that.  The user now wants the rule to number the drugs, allow the user
to select a number, and have that drug populate the Med field.  My rule
is a Cus-Defined Keyword.  The keyword calls a macro, which contains all
the code for the rule.  The syntax checker will not allow me to move
anything to @t.PHA.RX.med.  I'm not sure this would work even if it did.
Any suggestions?

Christina - This is one reason that I went with the MIS allergens from FDB - 
everything is based on NDC#. If you use FDB and are 5.5 or greater, I would 
advise changing. We've run into this problem with Robitussin AC (with 
Codeine) and Codeine allergies. If you do not have the correct class under a 
drug (48:08   ANTITUSSIVES
48:16   EXPECTORANTS) and have at least one of these classes under 
Codeine (we only had (28:08.08OPIATE AGONISTS), you won't be flagged. 
Same thing for duplicate checking. I know that this is absurd, but believe 
me, you either have the classes for you generic names correct, or you have 
the potential for errors in allergies and duplicate checking and Meditech is 
not going to fix this other than the fact that they have if you go to using 
MIS allergens. I'm trying to convince them to use the formulary duplicate 
checking too, but so far they don't seem to want to do that. Also, the AHFS 
classes have changes each year, so each generic name needs to be checked 
each time the AHFS book comes out.

We have a scenario where one of the patient allergies is Morphine HCL.
During order entry, the pharmacist enters an order of Morphine sulfate
but she did not get flagged at all. The allergy was entered as a
Generic. I checked the drug dictionary for MSO4, obviously the
Ingredient is Morphine and on the Ingredient Allergy field, one of them
is Morphine (F05). I called MEDITECH and asked them how come we did not
get the alert. This was their response:

Pellerin,Tracie (MEDITECH) - Mar 22, 2007 - 2004 EDT:  The generic
associated as an allergy is not the one associated to the drug MORP0050
that was ordered.  If you would like to capture all allergies associated
with Morphine, the allergy should be entered as a class allergy rather
than a generic.

Our Director of Pharmacy does not fully agree with MEDITECH. She
believes that the user should still have gotten the flag as Morphine
(regardless of how the allergy was entered, generic vs class) is clearly
the main Ingredient and is also defined in the Ingred Allergy. If
Pharmacy goes by Class Allergy, it's good in a way that it checks
everything but at the same time they would get unnecessary alerts which
can de-sensitize the user of these flags. Most of our pharmacists select
the generic allergy.

I have emailed MEDITECH again about how our pharmacy feels but hasn't
gotten any response yet. Has this happened to you? How did you resolve
the situation? How do you enter the allergy, generic or class?

We do. But we don't use the Meditech product for BMV.  We use CareFusion for 
BMV (CareMed) and we use the same product for Lab (CareCollect).  They both use

All I know is that in similar cases, if I re-code the problem report as
a brand new report, from scratch, then it works OK...Whatever hidden
left-over inefficient code gets solved if you code it from scratch.

We have the most bizarre issue since 5.5 update.

We have two reports (both call the exact same fragment), both supply the
exact same info for the select (verified by %Z.ddc)

I run one report - the fragment runs fast (5 seconds) (this report is
much more complex than the 2nd) I run the other report - easily a minute

I can even hard code the fragment with the select (passing @urn from
ADM) and run it by itself. Runs about 5 seconds.
I use the "slow" report to run the hard coded fragment, it hits the
fragment right away, but takes well over a minute to process it (even
though the select is already coded).

Other FYI: The fragment is pulling the patient's allergies using
meditech's mar allergy routine:
%PHA.MAR.allergies(patient,70,"",1,"",2)


I called Meditech's NPR and since have opened a task.  They said the
slowness has been reported before but didn't have a solution.

Has anyone else seen this issue?  Any suggestions would be greatly
appreciated. I just can't fathom why the same fragment (with hard coded
selects) would run so much slower when called from one report versus
another.

You made me curious about this and I found this bit of information in
the Summary for MIS/UNV training:
Customer Defined Select Screens - Selection screens are pop-up screens
containing radio buttons or check boxes used to answer customer-defined
queries.  The created queries are enclosed in frames instead of the
traditional question/response format it is in a pick list format.  The
type of queries used would have to be either a Group Response or Y/N
type. Currently, the Select Screens are only used in the Ambulatory
Order Management (RXM) module.

I've been using Boston Software Systems Workstation software for
scripting for two and a half years.  This product alone has saved us
countless hours of keying (LIVE and TEST for 9 facilities!!!).  Last
Spring I wrote a script to copy Scheduling dictionaries - it was very
time consuming and demanding!  Although I haven't used UNISON on those
Scheduling dictionaries I have written "Syncs" for a number of simpler
dictionaries but concentrated on MIS User and Preference Cards.  I'm
having a blast writing these and working with support on how to
accommodate things I want to do - multiple field lookups,  the ability
to change all or part of a field so that I can copy dictionaries from
one facility to another and more.

Don't Be Allergic to the New MAGIC Allergy Model

Abstract:  The new MEDITECH MAGIC Allergy Model 1B provides many benefits in
ease of use to the end users and in safety to patients.  The conversion
process for a hospital using the earlier allergy model can appear to be
intimidating.  This presentation will provide an overview of the whole
process and then describe each step of the process in detail as it was
implemented at Newman Regional Health in Emporia, Kansas.  Screen shots and
lessons learned will be provided, along with a list of changes that were
required within all modules for the implementation to be successful.

Jeff Thompson, R.Ph. is a Pharmacy Project Manager / Pharmacy Informatics /
Staff Pharmacist at Newman Regional Health, Emporia, Kansas. He has been in
that position since June 2005.  He is currently Project Manager for the
hospital-wide bedside medication verification system, laboratory collection
system, blood transfusion system and vital sign collection system.
Previously, he was a Director of Pharmacy for 13 years at a small rural
hospital in Kansas.

Move to the new MAGIC 1B allergy model.  There is no more class/generic/drug
allergies.  There is one dictionary in MIS that would have caught it. We
moved to it last fall and it is so much easier to use.  Another bonus is
that Nursing and PHA and ER can share the same database.  Call if you have
questions.

As an aside, we are 5.5.2 and I don't know if 5.4 can move to it but you
might check.

We have a scenario where one of the patient allergies is Morphine HCL.
During order entry, the pharmacist enters an order of Morphine sulfate but
she did not get flagged at all. The allergy was entered as a Generic. I
checked the drug dictionary for MSO4, obviously the Ingredient is Morphine
and on the Ingredient Allergy field, one of them is Morphine (F05). I called
MEDITECH and asked them how come we did not get the alert. This was their
response:

Pellerin,Tracie (MEDITECH) - Mar 22, 2007 - 2004 EDT:  The generic
associated as an allergy is not the one associated to the drug MORP0050 that
was ordered.  If you would like to capture all allergies associated with
Morphine, the allergy should be entered as a class allergy rather than a
generic.

Our Director of Pharmacy does not fully agree with MEDITECH. She believes
that the user should still have gotten the flag as Morphine (regardless of
how the allergy was entered, generic vs class) is clearly the main
Ingredient and is also defined in the Ingred Allergy. If Pharmacy goes by
Class Allergy, it's good in a way that it checks everything but at the same
time they would get unnecessary alerts which can de-sensitize the user of
these flags. Most of our pharmacists select the generic allergy.

I have emailed MEDITECH again about how our pharmacy feels but hasn't gotten
any response yet. Has this happened to you? How did you resolve the
situation? How do you enter the allergy, generic or class? 

We are Magic 5.4 and we have First Data Bank.

We used Pyxis' Veri5 BMV for 4 years and just recently switched to
Meditech's BMV (Pyxis no longer markets or supports Veri5).  Because we
were using an interface between Pyxis and Meditech, we had to modify how
we enter Pharmacy orders, including not using some of Meditech's
functionalities.  We were also not able to implement bar-coded IV labels
until we went with Meditech.  Since Meditech had their own BMV product
(which came out after we had already signed with Pyxis), they were not
willing to make any modifications for us.  We really like the
integration of using the Meditech eMAR and BMV because now nurses can be
prompted with lab values during med administration (among other things)
and they can submit requests for holds, etc. without having to call us.
I would caution you to be sure you don't want to use Meditech before you
sign with someone else.  Kathy

[MEDITECH-L] C/S Use of Handhelds for Ancillary Staff

I've never tried this, but it strikes me that, if you set up one report as the 
"master" and all the others as fragments of that report (run in the trailer 
region), you ought to be able to get what you're looking for.  If you want to 
select from various reports, you'd have to set up a select variable(s) with an 
IG operator, and then use the selection on line checks for each fragment.

You might try using 8. Edit Elements from the process reports menu.  I've set 
up some small selects there.

I've never tried this, but it strikes me that, if you set up one report as the 
"master" and all the others as fragments of that report (run in the trailer 
region), you ought to be able to get what you're looking for.  If you want to 
select from various reports, you'd have to set up a select variable(s) with an 
IG operator, and then use the selection on line checks for each fragment.
