Not sure why you would need an add-in for this.
If you create several alerts for the same entry, and adapt your WHEN rules 
you have exactly what you need.


As for your 2nd "request", that's something that is not possible with 
Servers Alive.  The only way I see that this could be done, is to create a 
mail-agent.   This is basicly an app that polls a mailbox and based on the 
content of the received mail issue the needed ssh/telnet commands towards 
Servers Alive and those will then put the entry into maintenance or stop the 
paging.

server goes down, it sends out a page to a primary admin. That primary admin
does not respond back or get the server up in a time limit, the servers
alive will send a new page out to an alternate contact (like escalation) and
keep sending up a chain of command until the server is brought up (each
having a time limit between escalations)

2. Any way for servers alive to take action based on some snmp event or
email alert -

server goes down, servers alive sends out an email to a primary admin,
primary admin replies to the email and somehow servers alive does an action
based on that email response - like stops sending pages or puts the server
in maintenance mode
