Having the icon in order to see the status is that "enough" to start with, or 
would you need a full interaction (meaning that you're able to get the full 
interface and make changes?)

When it runs as a service there is no way to interact with it.   There was also 
a critical problem that I no longer recall but could probably duplicate.   
Furthermore on both my Windows XP and my Server 2008 machine when run as a 
service I somewhat frequently get the Method ~of ~ errors along with Pings 
coming back 0%.  This requires a restart of the app.     I thought it was 
windows 2008 specific until one day another admin ran it as an app on my XP 
machine (out of habit) and I got the same problem.

We received some remarks from users that Servers Alive was not able to run on
Windows 2008.
We would like to get some additional info on this. 
We did some testing and it seems to run as service (and be able to do checks too
:-))....
There are indeed some issues with for example the GROUPS that aren't
loaded/saved correctly due to the location of the GROUPS.GRP file.
