The title must be exactly the same as what is on the first page,
or it will be automatically rejected. The authors must also agree, but you
can add extra names, so I added William A Stein, since I assumed some
might search for his name. There is no option for keywords but
you can select 1 category.

Under "sales region" I selected "US and international sales". That is the
best I can do. Let's wait until it shows up on the US amazon store.
If it doesn't show up at the UK amazon store, I'll look into seeing if I can
register a complaint.
