I had a custom bike built around te Rohloff, so no spacing problems. I believe you may need to use a chain tensioner if the bike is not properly set up for the hub.
I had maintenance problems with my Rohloff. The seal blew after a very short while. Rohloff was in between U.S. distributors at the time and it took my LBS months before they were able to get Rohloff approved oil (LBS happened to have a seal in stock). I wound up giving the bike away at a promotional event for a local not for profit bike store.
