Are there other specifics--ie, rake, rack mounts, etc? I ask because I've 
purchased used forks for projects with good success. Depending on your 
steerer tube length requirement, I may even have one shortly. Much cheaper 
route. Otherwise, I think you will probably be looking at $250 bare 
minimum, and up from there. Tom Matchak has done a number of custom forks, 
if I recall. Jeff Lyon as well, though I think mostly batch jobs. With 
forks, I would make certain of the builder's experience level before 
worrying about price.

Right now nothing on eBay with a fork crown like you want, but there is

If you need rack mounts, you can have a builder put them exactly where you 
want--could probably even re-rake it. 

I got lucky with my 26" project. I had a NOS Riv 700c fork, unpainted. Just 
moved the canti posts, and now has tons of clearance for fat 26" tires.
