I hate to say this here, but none of the Rivendells would be my first choice.
A custom bike is ok if the buyer is mega-rich and doesn't mind waiting a long time. Otherwise it's a lot of money to spend ($4000-5000+) and a long time to wait, and if/when the person loses some weight (assuming that's the goal), the custom features suited to a big person may not be ideal for the skinny person. I like Eric's idea of a cheapish overbuilt MTB, probably with a wheel upgrade. Or, if $1500-2000 is feasible, a Surly Instigator built at a qualified LBS. The Instigator (with Pugsley front fork/wheel) has been my main ride recently, and it has a lot going for it as a bike for a bigger person. It has a 1.5" down tube, gussets, big tire clearance, a disc brake option, etc, etc...
