Bob Brown made a very nice super strong custom for the LFOAB when he weighed well over 400 pounds. (he weighs well under 200 pounds now and lost 330 plus pounds via cycling and diet changes) I would also consider Bob for this if the comotion bike is no longer production and custom is the way to go. Here is Scotts story in pictures: http://www.bicycling.com/bke/slide/home/1,8155,s1-1-81-0,00.html and at least the fork on his bike that Bob made is under custom fork crown on this page: http://www.bobbrowncycles.com/custom.htm
Scotts (defunct) blog is here if you are not familiar with it: http://istanbultea.typepad.com/ Scroll back to the beginning for the full story. I had the pleasure of hanging out with him and his family a few years back in Minneapolis and riding around with him, he is a great guy. But anyhow. The comotion is probably the best bet. I suspect their customs would be cheaper than Bob's, but depends on what your goals are. I think almost any custom builder COULD do the job. But alot were not super helpful to scott when he started his journey.
