I can't replicate the problem. I just edited qmail-scanner-queue.pl and put a "sleep 1200" just before clamdscan was called (thus emulating Q-S sitting there for ages waiting for clamdscan to return). After 20 minutes (the MAXTIME setting in Q-S) the alarm triggered and Q-S exited "Maximum time exceeded. Something cannot handle this message." and a "451 qq temporary problem" to my telnet session where I have manually entered the message. No message sent - as expected I then did the same thing, but this time only waited a minute before manually killing the SMTP session (emulating your situation of the client giving up). The qmail-scanner-queue.pl process continued existing for that 20 minutes, then exited as above - no message was sent - as expected. Whoah! That's a tad short. That really gives them very little leeway for remote systems to process under load. I still think something's wrong with your clamd - or you actually have loaded systems that just can't process large messages in such a timeframe. I know you said you have high-end servers - but "high end" doesn't mean anything if they are under "high load"... What kind of load are they experiencing? What is the load average on them? If it usually lies below 1.0, then that's great - but if it generally lies above 1.0, then that means they are under load.
