Maybe you should start checking the silly stuff like paths and permissions?

Could be the issue is hiding that way.  Have you looked at all the logs to
be sure everything is processing the way it should be?  Any oddities in
there?

I believe that qmail-scanner does not do SA scanning if RELAYCLIENT is set, it should still do the virus scanning if you have that enabled though.

Sorry, I don't run qmail-scanner anymore so I can't tell you for sure.
