If it's in the smtpd run file, it should work. Everything passes through smtpd, even mail that's a local domain.
Maybe you should start checking the silly stuff like paths and permissions? Could be the issue is hiding that way. Have you looked at all the logs to be sure everything is processing the way it should be? Any oddities in there?
