I fixed a problem like this for a cable company. Some of their cablemodem
customers got infected by a virus that loaded down the cable company mail
servers.

The first thing to do is clean the infection. You must identify the systems
that are infected and clean them. Once the traffic is no longer being
generated, just clean out your queue. In my case, I had to rely on the
customer to clean their PCs of infection. Until that was accomplished, I
blocked their network traffic. The customer cablemodem connection was
useless because they were being blocked until their systems were clean. The
company mail servers no longer received the infected emails since they were
blocked by iptables on the server. That motivated the customer to clean out
the viruses. It is useless to try and filter emails from infected systems.
The infection must be cleaned, and infected systems should just be
completely BLOCKED.
