It doesn't - just like the other patch mentioned. If a mail message comes in
for >1 recipient, then SA cannot do per-user configuration. The only way
that could be achieved would be to split the recips into an array and run SA
for each of the recips - can you say major load?

If people desperately want this feature (for the <1% of messages their site
receives with >1 recip...), then they will be desperate enough to realise
they can do it by simply putting another "raw" Qmail server in front of the
Qmail-Scanner server. As everyone knows, Qmail always delivers via
one-recip-per-SMTP session...
