Hi Steve,
It certainly seems like it's the mouse pointer not following the VoiceOver 
cursor. Go to the Navigation category of VoiceOver Utility and make sure 
"Follows VoiceOver Cursor" is selected for the Mouse Pointer pop-up. Let's see 
if that was the only issue.
