Start by running this in the xserver directory, to get debugging symbols for the main server code. You can also rebuild driver/xf86-video-r128 this way to get symbols for the r128 driver. But maybe, before you do that, send us the full /var/log/Xorg.0.log for the machine when you start X without an /etc/X11/xorg.conf file. The file you sent below is clearly incomplete. It misses information about the xorg.conf file used, or about modules loaded. I don't know why/how some lines got removed, but they may contain the explanation for the error you're seeing. FYI I've appended the log file I get on a similar machine (not exactly the same r128 chipset though), where X is running fine.
