I heartily recommend PostScreen, designed to detect and reject compromised 
BotNet hosts. 

Configured with a reliable RBL blacklist (say zen.spamhaus), it can reject over 
90 percent of bad senders, before they
even connect with your mail server.

I see perhaps 4 false positives a year, where it rejects something I wanted.
