We're using a spamassassin based solution called maia mailguard, along with policyd v1 and clamav, to manage our spam. The policyd component does greylisting and other policy measures, while mailguard provides a web-based spam management interface. We also supplement spamassassin and clamav with some extra rule sets which enhance effectiveness.
