Yesterday I've seen exactly the same problem on our Polish forum and the
jtag_khz 1000 helps, which suggests that there is some problem with
adaptive clocking. Are you sure you've forced the speed (jtag_khz as the
LAST command, not jtag_rclk, no other commands in reset scripts etc.)?
